Activity Director Resources and CME Activity Application

To host a new Professional Education program at Hospital for Special Surgery (whether CME or non-CME accredited),
please complete the Professional Education Program Request form (doc) and email it to professionaleducation@hss.edu. Once your Request form has been reviewed, you will be contacted with additional information on how to proceed. If you are applying for a reoccurring CME activity (i.e., Annual Holiday Total Knee Course), a traditional CME activity, or Internet CME resource, you will need to also complete the CME Activity Application and Planning Form (pdf) and email it to professionaleducation@hss.edu. Your application will be reviewed within 30 days of receipt and scheduled for approval by the hospital's Continuing Medical Education Committee. Before completing either of the forms, we encourage you to review the Activity Director's Resource Guide (pdf) on how to plan, design, implement, and evaluate a non-CME or CME activity and to contact the Director of Professional Education at 212.606.1834 or GoldbergA@hss.edu to discuss your proposed activity.
